ADHD or Anxiety? How Assessment Clarifies Symptoms

Difficulty concentrating does not automatically mean ADHD. Anxiety can also pull attention toward worry, threat and physical tension—and ADHD itself can create years of stressful consequences.

Why this matters

Quick answer: Clinicians distinguish ADHD and anxiety by examining when symptoms began, where they occur, what triggers them and whether attention improves when worry decreases. Both conditions can also be present.

Research Spotlight

Similar symptoms, different drivers

  • Timing matters
  • Triggers matter
  • History matters
  • Both may occur

How the patterns can look similar

Either condition can contribute to procrastination, forgetfulness, restlessness, sleep problems and inconsistent performance. A brief checklist may produce a high score without clarifying the cause.

Clues a clinician considers

More consistent with ADHD

A long-standing pattern across settings, difficulty regulating attention even during calm periods, chronic time-management problems and evidence from childhood may support ADHD.

More consistent with anxiety

Concentration problems that intensify around worry, evaluation, uncertainty or physical arousal may point more strongly toward anxiety.

Why co-occurrence matters

If both are present, treating only one may leave important difficulties unresolved. Recommendations should prioritize the problems causing the greatest impairment and account for safety, preferences and existing care.

What assessment may include

  • Clinical and developmental interviews
  • ADHD and anxiety rating scales
  • Review of school, work and relationship functioning
  • Sleep, mood, trauma, substance and medical screening
  • Collateral information when appropriate and available
